This worksheet helps us collect information about your educational plans and customize your financial aid package to your unique situation.
ASSET VERIFICATION WORKSHEET
This worksheet helps us collect information about your assets/investments. Used only when the information on your FAFSA suggests that you may have financial or investment assets that must be included on your FAFSA.
DEPENDENT VERIFICATION WORKSHEET
This worksheet helps us collect information about your family size, the number of family members attending college, and other miscellaneous untaxed income that normally would not appear on your federal tax forms or W-2's.
FINANCIAL SUPPORT WORKSHEET
This worksheet helps us document non-traditional forms of support or income for rent/mortgage, utilities, grocery, and transportation expenses. Used only when the information on your FAFSA suggests that your (or your family's) combined income was unusually low for the number of people in your family.
INDEPENDENT VERIFICATION WORKSHEET
This worksheet helps us collect information about your family size, the number of family members attending college, and other miscellaneous untaxed income that normally would not appear on your federal tax forms or W-2's.
SPECIAL CIRCUMSTANCES APPEAL
This form is intended for students requesting a recalculation of financial need based on special or unusual circumstances. This appeal is only appropriate if ALL of the following are true.
  • Your 2009 financial information will be significantly different than it was in 2008.
  • Your 2009 financial information is different than the information you provided on your 2009-2010 FAFSA.
IMPORTANT: Submitting an appeal does NOT guarantee that your appeal will be approved and your financial aid eligibility will adjusted.

MAXIMUM CREDIT APPEAL
This form is intended for students who have reached a total of 80 attempted credits (including transfer credits). This appeal is required in order to continue eligibility for financial aid.

IMPORTANT: Submitting an appeal does NOT guarantee that your appeal will be approved and your financial aid eligibility will be extended.
FINANCIAL AID SUSPENSION APPEAL
This form is intended for students who have received a notification of financial aid suspension. Students who have not met the Satisfactory Academic Progress Standards can appeal to a committee to restore financial aid eligibility for future semesters. Please note: If you have been placed on academic suspension, see the Academic Suspension Information in the Registration and Records section.

IMPORTANT: Submitting an appeal does NOT guarantee that your appeal will be approved and your financial aid eligibility will be reinstated.
SATISFACTORY ACADEMIC PROGRESS POLICY
Students applying for or receiving financial aid at the College of Western Idaho must meet the college's standards of satisfactory academic progress in order to first establish, and then maintain eligibility. This document describes CWI's financial aid satisfactory academic progress policy in detail.
CONDITIONS OF FINANCIAL AID OFFER
Students applying for or receiveing financial aid at CWI must read and understand the conditions of any financial aid offer issued by CWI and is referenced in all award notifications.
Registration and Records
ACADEMIC APPEAL FORM
This form is used to exercise your right of appeal to academic policies, requirements, student account charges, etc. Please carefully read and follow the directions in the information section of this form. When complete, submit form and all required materials to One Stop Student Services. This is NOT the appropriate form for appeals related to Financial Aid and Satisfactory Academic Progress.
ACADEMIC SUSPENSION INFORMATION
These documents are used in the event that you are placed on Academic Suspension. Please download each form and contact One Stop Student Services to set up an appointment to meet with an Enrollment Specialist. Please note: if you have been placed on financial aid suspension, see the Financial Aid Suspension Appeal in the Financial Aid section.
ADD/DROP FORM
This form is used for registering, adding and dropping courses.
ADDRESS CHANGE FORM
This form is used to change your mailing address only. If you need to change your legal address (with changes that could affect your residency status such as moving to a different county or state), please complete a Certificate of Residency Form.
BACHELOR'S DEGREE EVALUATION REQUEST FORM
This form is used if you have earned a Bachelor’s degree from another institution and have taken courses there that will count as prerequisites for your current degree, program, or a specific course at CWI. NOTE: You must be admitted as a degree-seeking student to use this form.
CERTIFICATE OF RESIDENCY FORM
This form is used to declare your legal county of residence. All students must complete this form each semester.
CHALLENGE CREDIT FORM
Use this form to request Challenge Credit. A course may be challenged with the permission of the instructional dean, department chairperson, and instructor by passing a comprehensive test for that course with a ‘C’ grade or better. You will be assessed 20% of the Idaho Resident Tuition for each credit acquired by either of these methods in addition to testing fees and any other tuition and fees.
COMMUNITY EDUCATION REGISTRATION FORM
This form is used to register for community education (non-credit) courses. You can also register for community education online.
COMPLETE WITHDRAWAL FORM
This form is used to completely withdraw from all courses. Plan to meet with a One Stop Specialist when you submit this form.
DIRECTORY HOLD/PRIVACY FORM
This form is used to withhold disclosure of your Directory Information under the provision of the Family Educational Rights and Privacy Act of 1974.
EXCEPTION TO ACADEMIC REQUIREMENTS FORM
This form is used to request exceptions to academic and degree requirements for graduation purposes. It is your responsibility to provide rationale and/or documentation justifying the exception requested.
GRADUATION APPLICATION FORM
This form is used to apply for graduation. You should apply at least one semester before you are ready to graduate.
GRADUATION APPLICATION CHANGE FORM
This form is used to update your previously submitted Graduation Application with a change in diploma name, major plan or class schedule change.
INCOMPLETE GRADE CONTRACT
This form is initiated by the instructor and must be completed when an incomplete grade is assigned.
INTERNSHIP FORM
This form is used to register for internship credit(s). All signatures must be completed before a student can register for an internship (293).
MAJOR CHANGE FORM
This form is used to change your declared major, catalog year and/or program advisor. Any of these changes may affect financial aid benefits, enrollment status, degree requirements, degree planning needs, graduation timeline, and other college-related business.
NAME CHANGE FORM - STUDENTS
This form is used to change your name on official school records. Documentation is required.
PRIVACY RELEASE FORM (PERMISSION TO RELEASE EDUCATION RECORD INFORMATION)
This form is used to authorize the College of Western Idaho to release your specified student information to individuals you designate. This form must be submitted IN PERSON with Photo ID.
REMOVE DIRECTORY HOLD
This form is used to remove your directory hold. (See Directory Hold/Privacy Form)
SELF-ADVISING REQUEST FORM
This form is used to request self-advised status. If your request is approved, you will not be required to meet with a faculty advisor prior to registration each semester. It is still recommended that you meet with an advisor the semester prior to graduation to confirm your eligibility to graduate.
SPANISH PLACEMENT VERIFICATION FORM
This document is used by students interested in obtaining Spanish placement. Students may take the Spanish Test at Boise State University’s Testing Services office, (208) 426-2762. Please note: Students must take this form with them to be filled out by a BSU Proctor. The form must then be returned to a CWI One Stop Student Services Office.
VERTICAL CREDIT FORM
Use this form to request Credit by Examination (CBE) for Vertical Credit. You will be assessed 20% of the Idaho Resident Tuition for each credit acquired by either of these methods in addition to testing fees and any other tuition and fees.
Student Employment
PRE-EMPLOYMENT DOCUMENTATION
In order to begin working as a CWI Student Employee, you must document that you are eligible to work in the United States and complete a tax witholding. You must submit your I-9 and W-4 forms along with your work-study Referral Form (signed by your work-study Supervisor) before beginning employment.
WORK-STUDY REFERRAL FORM
This form is used to authorize your employment in a work-study position and to notify your supervisor, Human Resources, and Payroll of your employment. If you have been awarded work-study funding, please contact CWI One Stop Student services to obtain a work-study Referral Form.
This form must be obtained in person from CWI One Stop Student Services - please bring picture ID.
WORK-STUDY TIMESHEET
Student employees should record their time hourly on a work-study Timesheet only. REMEMBER: Student employees are only allowed to work a maximum of 19 hours per week (up to your total work-study award) and may not work during your regularly scheduled class times.
